Menopause is associated with a rapid decline in oestrogen levels, which plays a critical role in maintaining skin structure and function. This hormonal shift contributes to what is increasingly recognised as hypoestrogenic skin syndrome, a distinct biological process that differs from intrinsic (chronological) ageing.

Oestrogen deficiency leads to a marked reduction in dermal collagen—up to 30% within the first five years along with decreased synthesis of hyaluronic acid and glycosaminoglycans. The result is diminished skin thickness, reduced elasticity, and increased transepidermal water loss. Clinically, this manifests as fine lines, laxity, dryness, and a loss of radiance.

In parallel, the epidermal barrier becomes compromised, increasing susceptibility to irritation, pruritus, and delayed wound healing. Altered immune signalling contributes to a pro-inflammatory cutaneous environment, while dysregulation of melanocyte activity often compounded by vascular changes predisposes patients to pigmentation disorders such as melasma and uneven skin tone.

Why does my skin change so much during menopause?

Menopause leads to a significant decline in oestrogen, a hormone essential for maintaining collagen, hydration, and skin thickness.
