Liposuction is a surgical procedure and simply means fat removal. Over the years liposuction has evolved a lot and gone are the days of traditional liposuction involving general anaesthesia leading to hospital stays, blood losses, various postoperative complications which at times lead to death.
Dr Jeffrey Klein, a dermatologic surgeon from California, USA, invented a type of local anaesthesia called Tumescent Anaesthesia in 1986 which revolutionised the world of medicine. It was now possible to perform liposuction totally under local anaesthesia.
Tumescent is an English word meaning ‘firm &swollen’. Tumescent anaesthesia involves injecting large volumes(2-4L) of diluted anaesthetic solution inside the target fatty pocket to be removed until the area becomes firm, hard and swollen,.
The anaesthetic drug inside the solution numbs the whole area, causes fat cells to swell up and breakdown and shrinks the blood capillaries in the area to minimise blood loss.

Rolls Royce of liposuction techniques! Also known as microcannular liposuction or Kleinlipo was again the brainchild of Dr Jeffrey klein.The procedure is carried out using very fine instruments or cannulas ranging from 0.9mm-2.3mm thick as compared to industry average of 6-8mm thick instruments. Most patients typically return to work in 2-4 days depending upon the area treated and amount of fat removed and need to wear postoperative compression garments for only 2-4 days after surgery except inner thighs and lower legs which require them to be worn for about 2 weeks to provide added comfort.

Vaser liposuction is one of the many modern-day liposuction techniques available to be done under local anaesthesia as a walk-in walk-out procedure avoiding hospital stays.
Its performed under local anaesthesia alone or sometimes combined with IV sedation depending upon the size of the treatment area. Vaser is a type of ultrasound-assisted liposuction that heat and liquefies the fat, which is achieved by inserting a cannula into that issue.
Once liquefied, the suction process begins using another cannula to pull the fat out of the body.

This achieves a very effective and long lasting anaesthesia of skin and subcutaneous tissue.
Microlipo under tumescent local anaesthesia has eliminated the risks associated with general anaesthesia and risks of excessive surgical bleeding thereby eliminating greatest dangers associated with older and other forms of liposuction. It minimises blood loss so effectively that vast majority of patients only lose about 15-30ml(1-2 tablespoons) of blood even during large volume liposuction.
A tiny tube called a microcannula, connected to a suction or vacuum, then sucks the tumescent fluid from the body, removing the fat.
Unlike traditional liposuction, where the cannula diameter is six to 10 millimetres, microcannula are smaller, allowing the fat to be removed in a slower and more uniformed manner. This results in a smoother finish, and the procedure is so minimally invasive it leaves almost no trace any work has been carried out. One of the main benefits of microlipo is the elimination of the need for pain medication after the procedure, as the effect of the local anaesthetic lasts for many hours.
Tumescent liposuction, when done correctly and in the right quantity, is proven to be the safest form of liposuction. This is because the anaesthetic solution shrinks the capillaries thereby reducing surgical blood loss. Complications arising from microlipo are thus extremely rare.
AFTERCARE
All patients must wear compression clothing after the procedure to speed up their recovery and minimise post-operative bruising. Inside thighs and lower legs are challenging areas from a recovery point of view, and such patients should wear their compression clothing for at least two weeks and rest with raised legs as much as possible.
After the procedure, compression apparel is applied, and the patient is informed about how to ensure that the area is compressed until two weeks after the procedure.
There is little or no bruising due to the specialised nature of the procedure and postoperative drainage.

COST
While the cost of liposuction for lipoedema is naturally a major factor in the study of an operation, it should not be the most important factor.
Several factors determine the costs of an operation, the most important being the amount of time and effort on the surgeon's part.
Dr Gupta will consider the number of areas being treated when agreeing on a cost for the procedure and whether all can be treated in "one sitting." The patient's size, area of treatment and expected degree of difficulty of the specific procedure are also considered.
In certain areas, the total cost will be affected by more time and attention than elsewhere.
